Over the past several decades, the change in the country’s market economy and the population growth have greatly affected the lives, activities, and the demands for heath care as well as impacted the healthcare system. Since 1994, the time when the United States abolished the trade embargo against Vietnam, the economic development and the growth of the healthcare system in the country have been extraordinary. Even though substantial investment has been obtained from the Vietnamese government, limitations remain in the physical facilities in the healthcare system as well as in the facility design. Designing a new general hospital is a complicated process and requires a lot of attention and knowledge from a design team. Generally, the design process consists of two phases: a planning phase and a design phase. The planning phase is extremely important and will decide the success of the general hospital. G D Kunders, an Indian professional hospital administrator and a healthcare facility planner, has stated “Failing to plan is planning to fail” (Kunders 2004, 61). The main objective of this dissertation, “HOSPITAL FACILITY DESIGN: A GUIDE TO SPACE PLANNING FOR A NEW GENERAL HOSPITAL IN VIETNAM,” is to produce a guide that will be a reference source for the Vietnamese government to improve the current standard for hospital design. As a reference source, the proposed guide can be used as a tool to assist Vietnamese architects and other professionals in healthcare design areas in improving the quality of the hospital planning and design in Vietnam. This dissertation also aims to be a teaching reference source for architecture educators to use in their instruction at universities in Vietnam and a learning resource to help architecture students understand requirements for spaces in a general hospital as well as how a general hospital functions so they can better plan and design it. The proposed guide is set in an arrangement that can help educators and students without strong backgrounds in hospital design to easily understand how to plan a new general hospital. The research methods used in this dissertation are (i) document reviews, (ii) qualitative research based on Post-occupancy Evaluation (POE), and (iii) comparative analyses. The findings are used to create a guide to space planning for a new general hospital to use to improve the current Vietnamese standards, TCVN 4470: 2012, General Hospital - Design Standard, in hospital planning and design for Vietnam. LGO Best Thesis 2018: Predictive Modeling Project at MGH

After the official MIT commencement ceremonies, Thomas Roemer, LGO’s executive director, announced the best thesis winner at LGO’s annual post-graduation celebration. This year’s winner was Jonathan Zanger, who developed a predictive model using machine learning at Massachusetts General Hospital. “The thesis describes breakthrough work at MGH that leverages machine learning and deep clinical knowledge to develop a decision support tool to predict discharges from the hospital in the next 24-48 hours and enable a fundamentally new and more effective discharge process,” said MIT Sloan School of Management Professor Retsef Levi, one of Zanger’s thesis advisors and the LGO management faculty co-director.

Zanger, who received his MBA and an SM in Electrical Engineering and Computer Science , conducted his six-month LGO internship project at MGH that sought to enable a more proactive process of managing the hospital’s bed capacity by identifying which surgical inpatients are likely to be discharged from the hospital in the next 24 to 48 hours. To do this, Zanger grouped patients by their surgery type, and worked to define and formalize milestones on the pathway to a post-operative recovery by defining barriers that may postpone patients’ discharge. Finally, he used a deep learning algorithm which uses over 900 features and is trained on 3000 types of surgeries and 20,000 surgical discharges. LGO thesis advisor Retsef Levi stated that “in my view, this thesis work represents a league of its own in terms of technical depth, creativity and potential impact.” Zanger was able to have true prediction for 97% of patients discharged within 48 hours. This helps to limit overcrowding and operational disruptions and anticipate capacity crises.

A group of faculty, alumni and staff review the theses each year to determine the winner. Thomas Sanderson (LGO ’14), LGO alumni and thesis reviewer stated that Zanger’s thesis showed  “tremendous extensibility and smart solution architecture decisions to make future work easy. Obvious and strong overlap of engineering, business, and industry.  This is potentially revolutionary work; this research advances the current state of the art well beyond anything currently available for large hospital bed management with obvious and immediate impact on healthcare costs and patient outcomes. The theory alone is hugely noteworthy but the fact that the work was also piloted during the thesis period is even more impressive. LGO has done a lot of great work at MGH but this is potentially the widest reaching and most important.”

Zanger, who earned his undergraduate degree Physics, Computer Science and Mathematics from the Hebrew University of Jerusalem, will return to Israel after graduation and resume service as an Israeli Defense Forces officer.

Specialised hospitals include trauma centres, rehabilitation hospitals, children's hospitals, seniors' (geriatric) hospitals, and hospitals for dealing with specific treatment needs such as Cancer ,Heart treatment and certain disease categories. Specialised hospitals can help reduce health care costs compared to general hospitals

Background: The value of medical registries strongly depends on the quality of the data collected. This must be objectively measured before large clinical databases can be promoted for observational research, quality improvement, and clinical trials. "A craft is a branch of profession that requires some particular kind of skilled work. In historical sense, particularly as pertinent to the Medieval history and earlier, the term is usually applied towards people occupied in small-scale production of goods. The meaning of craft and its values are ever-changing with development of new techniques and methods. Craft village is a one-stop craft destination which offers visitors a unique variety of craft-related activities and programmes. It is intended to be a “must visit" tourist destination. In this village, visitors will learn about Nepali handicraft through craft demonstrations & hands-on interactive sessions. Visitors will experience the interactive handicraft making besides viewing the techniques in which they make such fine masterpieces. It also accommodates various trading units. This project intends to provide spaces with forms and functions to foster the development of crafts that Nepal is known for. The art and craft gallery housed in its premises displays wide collections of handicraft products/artefacts based on time-honoured Nepali craft traditions. The core concept of the Craft gallery is education and highlights the craft heritage of Nepal and local handicraft production technologies. There are many handicrafts promoting agency in Nepal. They are private, semi-government and government agency with different scope and scale. But they lack adequate space which truly acknowledges the production, promotion and display of crafts. The project has opted for traditional principles in architectural design –character of a traditional settlement (tole)."

With this aim in view, several efforts were made at rural development to better the socioeconomic lot of villagers from ancient times to British period Rural Habitat is an important national development agenda. In the decades following Independence many economic and infrastructural efforts have been channelled to improve rural areas and bring them into the mainstream of development. Nevertheless, the physical aspect of development has never been given due attention. The main purpose of choosing Basahi Akbalpur is to understand the difference in Architecture of developed Villages and undeveloped villages.This study deals with built and unbuilt areas in the villages and to understand how the Architecture, Planning, Lifestyle has changed in the Past Years. It also deals with the Social, Economic factors that affect the society.Also to study the various aspects that helped in the development of these village The Basahi Akbalpur as the study of tradional Houses or Dwellng units in Basahi Akbalpur .There varities of Traditionla Dwelling Unit (Kutcha Houses) ,in which variety of materials has been used. The key information ofthe research seek is on village and housing types, forms and development characteristics. To this end, the research applies multi-methods which include direct observation, structured and semi-structured interviews, archive and secondary information. The research fmds that the Basahi community are not unlike other villagers in Azamgarh. Ownership to land is priced, family ties highly regarded and that there is strong attachment to the village. With respect to the physical aspects, the growth and development of Basahi village are found to be influenced by factors of land inheritance, topography and increasingly important, access to roads. House type and building material used are also changing as the community in general adopts more modem house design and material. Neurosyphilis in the modern era: Literature review and case series

The term of neurosyphilis (NS) refers to infection of central nervous system by Treponema pallidum. Classically, it has been divided into early (meningitis, meningovascular) and late forms (general paresis and tabes dorsalis). The availability of penicillin and high sensitivity of Treponema pallidum to this antibiotic has led to a widely held perception about rarity of syphilitic forms with central nervous system involvement. However, patient can exhibit atypical clinical presentation. Recently different clinical cases with autoimmune encephalitis-mimicking presentation or atypical movement disorders were described. In this article we presented clinical case series with different clinical and MRI presentation and discuss diagnostic and treatment challenges. During our screening period at neurological department we revealed 6 NS cases. Three of them have an atypical presentation. The first patient was misdiagnosed as acute disseminated encephalomyelitis, the second patient had hippocampal sclerosis and epileptic seizures. Another patient had cognitive decline and autoimmune encephalitis-like MRI lesions. We put an emphasis on widening of indication for lumbar puncture and NS tests in patients with syphilitic anamnesis and neurological manifestations.
