New center for transgender health opens on uf health jacksonville campus, center offers help for patients with gender dysphoria.

JACKSONVILLE, Fla. – UF Health Jacksonville has opened its new Center for Transgender Health with plans to offer gender affirmation services within a comprehensive care approach.

The announcement made on Tuesday comes during Transgender Awareness Week.

The Center offers expanded surgical services, such as gender affirmation surgery and facial feminization, with a comprehensive care approach involving primary care, psychiatry and endocrinology.

Benny K. Tan, MB, BCh, BAO (MD), medical director of the newly established Center for Transgender Health, is currently accepting new patients and is working with primary care physicians and local organizations that support the LGBTQ+ community to inform patients of these services.

“I believe offering these unique services will break down the barrier of inaccessibility for patients with gender dysphoria in our area,” Dr. Tan said. “As we diversify health care systems nationwide, it is important patients receive safe, high-quality care and providers become more aware of this growing need.”

Before you start treatment, you will meet with at least one member of the TISCC medical team — a doctor or nurse practitioner — and a member of the TISCC behavioral health team, such as a social worker or psychologist. You'll have a complete medical evaluation to make sure that your treatment risks are identified and addressed. Before a surgical procedure, you will be seen by the appropriate TISCC surgical team. Evaluation of your mental health ensures that any mood or mental health concerns are reasonably well managed before you start treatment.

Each person is different. Your providers will look at your specific case to come up with the best recommendations for you. Your health care team will work with you during your treatment and make sure your expectations are realistic. Your team wants to make sure your goals are being met, any risks are managed and your questions are answered.

Due to Florida Senate Bill 254, there are new requirements for prescribing Gender-Affirming Hormone Therapy (GAHT) in Florida.

All FL members will start with a telehealth video visit with one of our Nurse Practitioners. During this visit, you and your clinician will review your medical history and discuss your care plan. Please note: you must have a virtual visit before your in-person visit.

After your virtual visit, you will have a required in-person visit with a physician/doctor to review and sign an Informed Consent form . FOLX will help you schedule this visit with us.

We have in-person hubs in:

Tampa, Tallahassee, Jacksonville, Orlando, and Aventura (between Fort Lauderdale and Miami).

After these visits, you'll receive your medications, as medically appropriate. The fee for an in-person visit is $40. In-person visits are brief and offered at a reduced rate to support members in meeting this requirement. Insurance cannot be used for these visits at this time.

Male to Female (MTF) in Miami, FL

Sex reassignment surgery (SRS), also called “gender reassignment surgery” (GRS) or “gender confirming surgery,” is a procedure designed to change transgender patients’ bodies so that their appearances better match their identities.

Although patients may choose to undergo SRS for a variety of reasons, many suffer from physical dysphoria, social dysphoria, or both. Physical dysphoria is a strong discomfort with the mismatch between identity and body, and social dysphoria is a strong discomfort with how other people perceive them. Patients desiring what we call “MTF” surgery include not just transsexuals, but anyone assigned male at birth who identifies as female, feminine, or a woman some or all of the time.

Breast Augmentation

The breast augmentation procedure provides patients with full breasts that project from the chest, and it can help transgender women achieve a more feminine physique. Breast augmentation for transgender patients is generally performed when hormone therapy fails to provide satisfactory enlargement of the breasts.

Since genetic male breast tissue is not much different from female breast tissue, transgender breast augmentation uses conventional breast augmentation techniques. This procedure is usually performed on an outpatient basis under general anesthesia and takes about one hour to complete.

Implant Type

Breast implants come in a range of materials, textures, and shapes.

Silicone implants consist of an outer silicone shell filled with cohesive silicone gel. New silicone implants, commonly called “gummy bear implants,” have been developed with a more highly concentrated silicone gel that reduces the risk of rupture and increases safety. Saline implants consist of a silicone shell filled with a sterile saltwater solution and can be filled after they are inserted into the breast pocket.

In addition to the material of the implant, patients also have the choice between smooth or textured implants and round or anatomically shaped (teardrop) implants.

During your consultation with Dr. Alexander, he will help you decide which incision location and implant type will best suit your needs.

Dr. Alexander will begin the breast augmentation procedure by making an incision for the implant to be inserted. The most preferable site for this incision is around the areola (the darker skin surrounding the nipple) because it allows for the most inconspicuous scarring. Dr. Alexander will then insert either silicone or saline implants, usually beneath the muscle for a more natural appearance and reduced risk of rippling. Once the implants are in place, the incisions will be sutured closed.

Following surgery, patients can expect to have some swelling and discomfort, which can be controlled with medication. Patients may also have a change in the sensation of their breasts immediately after surgery, but this usually resolves with time. Most normal activities can usually be resumed within a few days of surgery.

Removal of Loose Silicone From the Breasts

With the ever-growing popularity of plastic surgery comes the advent of risky and often illegal techniques to achieve a desired look. “Pumpers” are non-medical persons who inject loose silicone into areas of the body that the patient desires to augment. Unfortunately, loose silicone can migrate to different areas of the body and cause infection, disfigurement, scarring, and even death.

For patients who qualify, Dr. Alexander can remove loose silicone from the breasts, and in a few months he can perform breast augmentation surgery to safely and effectively achieve fuller breasts for the patient.

Brazilian Butt Lift for MTF Transgender Patient

This procedure adds curves to your figure by enhancing your rear end using your own fat tissue. The benefit of the Brazilian Butt Lift is that it removes unwanted fat from other areas, purifies it, and then transplants it to plump the butt. The Gender Reassignment Controversy

When people opt for surgery, are they satisfied with the outcome.

In an age of increasing gender fluidity, it is surprising that so many find it difficult to accept the gender of their birth and take the drastic step of changing it through surgery. What are their motives? Are they satisfied with the outcome?

Gender may be the most important dimension of human variation, whether that is either desirable, or inevitable. In every society, male and female children are raised differently and acquire different expectations, and aspirations, for their work lives, emotional experiences, and leisure pursuits.

These differences may be shaped by how children are raised but gender reassignment, even early in life, is difficult, and problematic. Reassignment in adulthood is even more difficult.

Such efforts are of interest not just for medical reasons but also for the light they shed on gender differences.

The first effort at reassignment, by John Money, involved David Reimer whose penis was accidentally damaged at eight months due to a botched circumcision.

The Money Perspective

Money believed that while children are mostly born with unambiguous genitalia, their gender identity is neutral. He felt that which gender a child identifies with is determined primarily by how parents treat it and that parental views are shaped by the appearance of the genitals.

Accordingly, Money advised the parents to have the child surgically altered to resemble a female and raise it as “Brenda.” For many years, Money claimed that the reassignment had been a complete success. Such was his influence as a well-known Johns Hopkins gender researcher that his views came to be widely accepted by scholars and the general public.

Unfortunately for Brenda, the outcome was far from happy. When he was 14, Reimer began the process of reassignment to being a male. As an adult, he married a woman but depression and drug abuse ensued, culminating in suicide at the age of 38 (1).

Money's ideas about gender identity were forcefully challenged by Paul McHugh (2), a leading psychiatrist at the same institution as Money. The brunt of this challenge came from an analysis of gender reassignment cases in terms of both motivation and outcomes.

Adult Reassignment Surgery Motivation

Why do people (predominantly men) seek surgical reassignment (as a woman)? In a controversial take, McHugh argued that there are two main motives.

In one category fall homosexual men who are morally uncomfortable about their orientation and see reassignment as a way of solving the problem. If they are actually women, sexual interactions with men get redefined as heterosexual.

McHugh argued that many of the others seeking reassignment are cross-dressers. These are heterosexual men who derive sexual pleasure from wearing women's clothing. According to McHugh, surgery is the logical extreme of identifying with a female identity through cross-dressing.

If his thesis is correct, McHugh denies that reassignment surgery is ever either medically necessary or ethically defensible. He feels that the surgeon is merely cooperating with delusional thinking. It is analogous to providing liposuction treatment for an anorexic who is extremely slender but believes themselves to be overweight.

To bolster his case, McHugh looked at the clinical outcomes for gender reassignment surgeries.

Adult Reassignment Results

Anecdotally, the first hurdle for reassignment is how the result is perceived by others. This problem is familiar to anyone who looked at Dustin Hoffman's depiction of a woman ( Tootsie ). Diligent as the actor was in his preparation, his character looked masculine.

For male-to-female transsexuals, the toughest audience to convince is women. As McHugh reported, one of his female colleagues said: “Gals know gals, and that's a guy.”

According to McHugh, although transsexuals did not regret their surgery, there were little or no psychological benefits:

“They had much the same problems with relationships, work, and emotions, as before. The hope that they would emerge now from their emotional difficulties to flourish psychologically had not been fulfilled (2)”.

Thanks to McHugh's influence, gender reassignment surgeries were halted at Johns Hopkins. The surgeries were resumed, however, and are now carried out in many hospitals here and around the world.

What changed? One likely influence was the rise of the gay rights movement that now includes transgender people under its umbrella and has made many political strides in work and family.

McHugh's views are associated with the religious right-wing that has lost ground in this area.

Transgender surgery is now covered by medical insurance reflecting more positive views of the psychological benefits.

Aspirational Surgery

Why do people who are born as males want to be women? Why do females want to be men? There seems to be no easy biological explanation for the transgender phenomenon (2).

Transgender people commonly report a lifelong sense that they feel different from their biological category and express satisfaction after surgery (now called gender affirmation) that permits them to be who they really are.

The motivation for surgical change is thus aspirational rather than medical, as is true of most cosmetic surgery also. Following surgery, patients report lower gender dysphoria and improved sexual relationships (3).

All surgeries have potential costs, however. According to a Swedish study of 324 patients (3, 41 percent of whom were born female) surgery was associated with “considerably higher risks for mortality, suicidal behavior, and psychiatric morbidity than the general population.”

I recently had a zero depth labioplasty done by Dr. Whitehead and Dr. Rodriguez. I’m 7 weeks into my recovery and because of their knowledge and sharing it with me have left few surprises. There wasn’t a question that was unanswered. Post surgery I had Dr. John’s cell number and if he didn’t answer his response was within the hour. During my 10 days of recovery near his facility I was visited every day. Yes a Dr. that makes house calls. Appeals Court Finds a Constitutional Right to Gender Reassignment Surgery

The 14th Amendment to the Constitution is truly a magical amendment. It was used to justify attempts to kick Donald Trump off the 2024 ballot. Through the decades, judges who want to play at social engineering have used it frequently to justify questionable law. 

It's even been invoked to bypass Congress to raise the debt limit.

Now, the Fourth Circuit Court of Appeals has decided that the amendment's "equal protection" clause means that state Medicaid programs have to cover gender reassignment surgeries.

The appeals court ruled that West Virginia's Medicaid rules on mastectomies are unconstitutional because they violate the "equal protection standard" by not covering mastectomies for gender dysphoria.

Yes, really.

The ruling also includes a North Carolina Medicaid case that the state government won't cover "sex changes." The Fourth Circuit nullified the state ban on gender change surgeries, citing the 14th Amendment’s guarantee of “equal protection of the laws.”

This was a court looking for an excuse to make law.

Judge Roger Gregory who wrote the majority opinion in Kadel v. Folwell (8-6) asked, “Is removing a patient’s breasts to treat cancer the same procedure as removing a patient’s breasts to treat gender dysphoria?” He continued, “There is no case law to ground this discussion nor obvious first principles.”

Wall Street Journal:

He is undeterred, and he concludes that gender dysphoria and transgender status are intertwined, so that such insurance exclusions are nothing more than a proxy for discriminating against gender identity. Then he goes further, finding that West Virginia’s and North Carolina’s policies also unconstitutionally discriminate based on sex. How so? Imagine, Judge Gregory says, an unidentified patient seeking a vaginoplasty. Is this a biological female with a rare birth defect? Is it a transgender patient? “By virtue of the fact that they are seeking a vaginoplasty, we know that they were born without a vagina,” he writes. “But we do not know what sex they were assigned at birth. Without that information, we cannot say whether the Plan or Program will cover the surgery.” Ergo, sex discrimination.

Gregory gets even nuttier.

The differences in coverage "is rooted in a gender stereotype: the assumption that people who have been assigned female at birth are supposed to have breasts, and that people assigned male at birth are not."

It's not a "gender stereotype." It's a biological fact. 

"No doubt, the majority of those assigned female at birth have breasts, and the majority of those assigned male at birth do not. But we cannot mistake what is for what must be.”

Not just a "majority." It's a universal biological fact with a tiny number of exceptions.

Treating different things differently doesn’t violate the 14th Amendment’s Equal Protection Clause, and jurists aren’t supposed to ignore the obvious. Writing in dissent at the Fourth Circuit, Judge Julius Richardson struggles to contain his exasperation. “The states,” he says, “have chosen to cover alterations of a person’s breasts or genitalia only if the person experiences physical injury, disease, or (in West Virginia) congenital absence of genitalia.” That determination does not turn on the patient’s sex or gender. “Christopher Fain—one of the plaintiffs below—received coverage for a hysterectomy based on a diagnosis unrelated to Fain’s transgender status,” the dissent says. Likewise, males with gynecomastia qualify for surgery coverage in West Virginia only “if they have physical symptoms, like breast pain,” meaning that isn’t a procedure done merely “to affirm a patient’s biological sex.”

The ruling that opened this can of worms was Bostock v. Clayton County, a case that "held that Title VII of the Civil Rights Act of 1964 protects employees against discrimination because of  sexuality or gender identity." Now, as a dissenting judge in Kadel v. Folwell,  Judge J. Harvie Wilkinson III is saying that this ruling could be a Roe v Wade  ruling for the transgender community.

“This is imperial judging at its least defensible,” he says, “What plaintiffs propose is nothing less than to use the Constitution to establish a nationwide mandate that States pay for emerging gender dysphoria treatments.”

He's not wrong. But getting the ruling past this Supreme Court would be a stretch.

As SC legislature winds down, bill banning transgender youth medical care returns to House

South Carolina’s state Senate voted to approve a House bill outlawing gender-affirming medical care for transgender youth. Now, it heads back to the House after the Senate designated amendments to the bill.

After a lengthy debate last week, state senators voted 28-8 to forward the bill to the House in the last days before the legislative session ends on May 9.

The bill would eliminate the use of puberty-blocking drugs, hormone therapy, and gender reassignment surgery for those under 18. It would allow puberty-blocking drugs for minors diagnosed with conditions like precocious puberty or endometriosis who use puberty blockers as treatment.

It would also ban the South Carolina Medicaid Program from providing coverage for minors and adults.

One of the largest changes from the Senate’s amendments was a provision requiring public school principals and vice principals to inform parents if a child reported identifying as a different gender from their assigned sex or using different pronouns.

Opponents of this bill argued this would be a forced outing and could cause problems for a child at home. Sen. Tameika Devine (D-19), for example, had an issue with the bill that it only addressed public schools and not private schools. She also argued that the bill revokes medical freedom.

“This bill isn’t showing love, it’s not showing compassion. It is actually malicious and is showing that we are not caring for all citizens,” Devine said.

Supporters of the bill argued that the goal of the bill was to protect children. Those against the bill argue that the bill would harm transgender children.

“The main reason for this bill is to protect minors from making life-changing decisions that can have harmful and irreversible effects,” Sen. Richard Cash, a Republican representing Anderson County, said last week.

But, a study from the New England Journal of Medicine shows transgender and non-binary youth who use hormones and puberty blockers show lowered signs of depression and anxiety.

If passed by the House, South Carolina would join 25 states to outlaw medical care for transgender youth, according to the Human Rights Campaign.
